This is a free clinic that serves mean, women, and children who do not have healthcare or the ability to pay for needed care. St. Luke’s provides basic primary and preventative healthcare services as well as health and wellness education. The clinic is staffed by volunteer medical professionals and is funded by tax deductible contributions. Please call is you have additional questions, wish to volunteer or to make a donation.

	Shut-off Protection Plan (SPP)

This plan provides year-round protection from shut-off and is available to customers with a household income at or below 200 percent of the federal income eligibility guidelines.
	The plan requires:
· Initial down payment of 10 percent of your total bill
· Participating in a budget plan that spreads out your annual energy costs into equal payments plus equal monthly payments on your past-due balance
	
	Medical Emergency Protection

If you have a qualifying, documented medical emergency, you could be protected from energy service shut-off for nonpayment of your energy bill for up to 21 days. You must provide us with written proof from a doctor or a notice from a public health official that service shut-off will aggravate an existing medical condition.

	Shut-off Protection: Active Duty in the Military

	If you or your spouse is the customer of record and is called to full-time active military service by 	the president of the United States or the governor of Michigan during a time of declared national or 	state emergency or war, you may apply for shut-off protection for up to 90 days and you may 	request an extension of this protection by reapplying.
 
You must provide verification of active duty status. At the end of active duty, you must notify us of your status. You still will be required to pay for the energy you used during your participation in this program. We will set up a payment plan for all past-due amounts to be paid within one year.

	Provides shelter to families and single mothers who are declared homeless for three weeks to 90 days. Meal services is provided which includes three meals each day, seven days a week. On-site client advocate available to help with any other needs.

	"AWARE, Inc., strives to eliminate domestic and sexual violence" while promoting social change and empowering survivors by offering shelter and services.
	
	AWARE can provide: 24 hour crisis services; Safety planning; Emergency shelter; Emergency needs; 	Legal advocacy; Support groups; Individual counseling; Help obtaining a PPO; Community referrals; Transitional housing; Children’s services; Community presentations; and 24 hour Sexual assault nurse examiner program (SANE).

Service: Prescription coverage and health care for very low-income disabled or elderly clients without Medicare (see next entry on Medicare D.) 
Medicare Prescription Coverage

Service: Offers prescription drug insurance to anyone with Medicare. The standard benefit requires a monthly premium (about $39/month), a yearly deductible of $310, then Medicare pays about 75% of the first $2,830 in prescription costs (2010 figures.) Folks with low incomes and low resources may qualify for government help paying the premium, deductible and co-pays. Annual enrollment period is November 15-December 31. For help comparing and/or enrolling in Medicare drug plans, or to see if you qualify for extra help, you may call the local Medicare/Medicaid assistance program (MMAP) at 1-800-803-7174.

Veterans Prescription Coverage and Health Care

Service: Prescription coverage with $8 or $9/month co-pay for VA-approved generic drugs. Drugs are mailed to veteran. 
Eligibility: military veteran of active duty, either disabled or with income (after medical expenses) less than $36,700 if single, less than $42,700 if married, and resources less than $80,000. Must have VA appointment at Ann Arbor or Jackson location; wait list may pertain. (2010 figures) 
Pharmaceutical Company Programs 
Pharmaceutical companies may offer “patient assistance programs” that provide 30-90 day supplies of medicines delivered to the doctor for qualifying low-income seniors who do not have drug insurance. Application process is lengthy and involves doctor’s staff as well as client to complete. Contact the national office of the drug company, or www.needymeds.com, for the eligibility criteria.

Jackson’s Reserve-A-Ride is available to the public on a first come first serve basis.  This service provides to door service to all residents of Jackson County. Reserve-A-Ride is available to the physically disabled by providing lift and ramps. Rides are provided by a twenty-four hour advance reservations with same day rides scheduled on a as available basis.

	Free nicotine patches, gum and lozenges are offered by the Michigan Department of Community Health for tobacco users who enroll in free cessation services through the Michigan Tobacco Quitline. The tobacco quitline can be reached 24 hours a day, seven days a week, at 800-784-8669. The quit program also provides personal coaches to help develop individualized quit plans. The coaches provide ongoing support with up to five telephone sessions around the caller's quit date. Spanish-speaking coaches are available. The program is available to all callers 18 and older, regardless of income or insurance status.
